Forms for Students

This form should be completed by all NEW F-1 students immediately after arriving in the U.S. to verify your U.S. address and phone and report your arrival.  This step is required to receive a continuing attendance I-20 and maintain status.

F-1 New Student Check in form.

Please request a verification letter by emailing occinternational@occ.cccd.edu.

We can verify the following information:

• Your major
• The dates of your attendance at OCC
• Number of units enrolled in each semester
• Fees paid or unpaid
• Dates on your I-20

We cannot verify your grades or academic standing. You must order official transcripts for proof of your grades and academic standing.

If you are not able to maintain and complete 12 units in Fall or Spring (or 6 units for your first Summer as a new student) you must be authorized for Reduced Course Load OR get Concurrent Enrollment.

If you specifically require a medical RCL, you may have your doctor (M.D., D.O. or Psychologist) in the U.S. complete and directly submit the Medical RCL letter template to your F-1 advisor.

RCL approval for any reason is required BEFORE dropping under 12 units.

​Transfer Out Request Form

Accepted to another institution? Congratulations! You must have your SEVIS record transferred to new institution once you are certain you want to attend that institution.  If you are waiting for other admission offers, please wait until Day 55 of your 60 grace period. Once you have decided, follow these steps to request the SEVIS transfer:

1. Complete the transfer out request form and upload a copy of your acceptance letter. 
2. Your transfer must be completed within your 60-day grace period for transfer- counted from the last day you attended classes at OCC.  If you miss the grace period, OCC will be unable to transfer your SEVIS record in active status! Please allow us about 5 days to process your transfer out.
3. U.S. Immigration regulations require that you must attend the next available session at your new school (no later than 5 months from the last day of attendance at your old school). 
4. If you plan to travel outside of the U.S. before classes begin at your new school, you will need to use the new school's I-20 to re-enter the U.S. 
5. If you are currently on OPT from OCC, you will also need to submit a copy of your OPT card and a letter of verification of employment from your employer to mhalley@occ.cccd.edu. Your OPT will end when we transfer your SEVIS record.

If you are offered an on-campus job and must apply for a social security number (SSN), please ask your job supervisor to complete the Employer letter.

Submit the letter to occinternational@cccd.edu and review detailed information about employment, SSN, and filing taxes on our Guides page, under 'Working in the U.S.'

If you are interested in learning more about OPT, you can sign up for an OPT workshop facilitated by one of our F1 advisors by clicking here HERE. Workshops are held monthly, typically online via Zoom.

After you attend the OPT workshop, you will receive instructions for the online filing of a post-completion OPT application, as well as a statement of understanding from your F-1 advisor which you will need to complete. When you are done preparing all your documents, please request an OPT I-20 and upload your documents using the OPT Document Check form for review.

Please email occinternational@cccd.edu to request an insurance waiver based on the criteria below:

WAVIER:  ONLY for use by government sponsored students (i.e. SACM or UAE), students covered under a parent/spouse employed by a U.S. employer, or students on active OPT insured through an employer's plan.

REFUND: Eligibility is subject to review; only available up to 45 days from coverage start or date of purchase, whichever is first. Students who have coverage as described above (waiver) or who have requested Termination - Authorized early withdrawal and have returned or will be returning to their home country immediately.

Sponsorship Form (PDF)

Use this form to complete an OCC admissions application or reinstatement application, etc. The form should be signed by you and your sponsor with a pen. The name of the account holder on the bank statement must match the sponsors signature on this form.

​International Student - Authorization to Release Records/Information to a Third Party (PDF)

This form is to be used exclusively by International Students only. If you are not an international student, please visit the OCC Enrollment Center -FERPA (Family Educational Rights & Privacy Act) for more information.

Use this form to authorize a third party (agent, parents, relative, or friend) to access your student information. In order to maintain student confidentiality, your OCC student information is not accessible to anyone other than you, unless you fill out this form. For example, if you want to have someone pick up your I-20, you will need to complete this form with their information. The authorized party will only be able to obtain your personal information in person (not over the phone) and with a valid photo ID. We must receive the form directly from you and you can withdraw the permission at any time by contacting the GEC in writing or in person.
