Required once per term, for initial request of certification.
You must be registered in classes before you submit this form.

Add/Drop Notification
Required any time you add or drop classes after you have submitted your initial certification request for the term.
VRE Authorization Information (Tungsten)
Students using VRE benefits will need to ask their VRE counselor to submit an authorization to OCC (in addition to submitting a Certification Request above). Please provide your VRE counselor with our information below.
